Preparing for your Physio assessment

You can have the assessment done in the comfort of your own home, or you can arrange to have it done at Heron Creek in our private Personal Training room at no additional cost. If you are doing it at home, read both sections below. If you are doing it at Heron Creek, talk to our reception staff to arrange the appointment first! Then, on the day of the assessment, review the second section below and show up a few minutes early. That's it!

Setting up at home for the assessment

Skip this section if you are doing the assessment at Heron Creek.

Technology

Device: Get a desktop computer, tablet, laptop or smartphone with a functioning camera and microphone.

 

Tripod or stand: If using a phone, set it up on a tripod or stable surface to provide a clear view of your body during exercises and assessments.

Internet Connection: Check that you have a stable and high-speed internet connection to avoid disruptions during the session.

Location

Well-Lit Room: Choose a well-lit space to enhance visibility. Natural light is ideal, but if that’s not available, use lamps or overhead lights to create good lighting.

 

Quiet Environment: Find a quiet place with minimal distractions. Inform family members or housemates about your appointment time to avoid interruptions.

On the day of the appointment

This section is important regardless of where you are doing the assessment.

Clothing

Wear comfortable clothing that allows for easy movement. Ensure the clothing is suitable for any physical assessments or exercises.

Note if you are doing the assessment at Heron Creek, you can pull down the blind for privacy!

Health Info

Medical History: Be ready to discuss your medical history, including any injuries, surgeries, or chronic conditions.

 

Current Symptoms: Note down specific symptoms or areas of concern you wish to address during the appointment.

 Payment

Payment method: Bring your credit card or debit card and have it handy when requested.

Receipt Submission: Make sure you understand the process for submitting your receipt to your insurance provider after the appointment.

Goals

Questions: Write down any questions or concerns you may have for your physiotherapist ahead of time.

​

Goals: Think about your treatment goals and what you hope to achieve from the session.
